Community Based Services

Passages’ clinical staff will help you navigate mental health challenges and life stress by developing a therapeutic relationship grounded in the belief that recovery from serious behavioral health challenges can and does occur.  Passages’ therapeutic services are based on the principles of recovery, which include respect, hope, individualized service plans, strengths, self-determination, non-linear healing, empowerment, peer support and responsibility.

Our clinical staff deliver services where you are most comfortable whether that be your home (age 12 and under), a community setting, or our office- the decision is yours. Let us guide you on your recovery journey.

Individual Services We Offer

Therapy

Our Master’s level therapists will work one-on-one with you utilizing evidence-based interventions.   You will work hand-in-hand with your therapists to develop a Recovery Plan that will help you achieve your mental health goals. 

Peer Support

Individuals enrolled at Passages can work one-on-one with a certified peer counselor who provides encouragement and advocacy. Your peer counselor can help you navigate the behavioral health system, provide educational resources, and wellness tools to empower you to not just survive but to thrive.

Care Coordination

Care Coordination services consist of assisting those enrollees with complex needs to develop an overarching care coordination plan that will include both formal and natural supports.  Our Care Coordinators will work with you to find community based resources and provided support in accessing services that meet your unique needs.  

Psychiatric Medication Management

Passages offers psychiatric medication management services to clients. This is a level of outpatient treatment that involves the initial evaluation of the patient’s need for medication, prescriptions for the medications, and ongoing monitoring related to the individual’s use of the medication by a qualified prescriber and nurse.

Through participation in Adult and Youth RTA, individuals can learn to channel overwhelming and challenging emotions by utilizing the visual and performing arts.

Therapeutic Group Highlight

BREATHE

BREATHE is a weekly meeting where you will be provided opportunities to build skills to navigate loss and your grieving process while in a safe space with peer support. You will be able to learn about the grief timeline, types of grief, navigating painful reminders, the B.R.E.A.T.H.E. model, as well as exploring your own emotions and needs associated with grief. If you are experiencing grief, please talk with your direct service provider about the opportunity to attend this group.

Adolescent Group Highlight

YOUth Group

YOUth group offers youth aged 13 and up the opportunity to connect with other youth in a place where everyone is welcome. Here they can build skills and share ideas on how to balance taking care of themselves, school, friends, and home life. Youth will have the freedom to participate in activities they enjoy both in the group setting and in the community.
