Peer Bridger/ Pathfinder

Peer Bridger

Peer Bridgers are certified peer specialists who use their training and lived experience with behavioral health challenges and hospitalizations to provide peer support for people preparing to discharge from the hospital to the community. A Peer Bridger will function as a role model, a supporter, a mentor, a teacher, an ally, and an advocate.

Peer support activities include:

  • Developing rapport and relationship
  • Promoting self-advocacy and direction
  • Sharing experiences, strengths, and challenges
  • Help you with the activities you identify as critical to successfully transitioning out of the hospital
  • Identifying and connecting to resources
  • Develop a recovery-based transition plan
  • Serving as a member of your treatment team to uphold your perspective

Peer Pathfinder

 

Peer Pathfinders are certified peer specialists who use their training and lived experience with behavioral health challenges and navigating systems to provide peer support for people who are in correctional facilities and preparing for release to the community. The Peer Pathfinder will function as a role model, a supporter, a mentor, a teacher, an ally, and an advocate.

  • Promoting self-advocacy and direction
  • Sharing experiences, strengths, and challenges
  • Identifying and connecting to resources
